Kindly submit the following information and document to the First Elder or Pastor no later than two weeks before the Sabbath on which you would like your baby to be dedicated:

1. Full Name of Father
2. Full Name of Mother
3. Baby’s Birth Certificate
4. Parents’ Contact Information

An interview will be scheduled by the First Elder or Pastor before the dedication date.
